A collaborative process which assesses, plans, implements, coordinates, monitors and evaluates options and services to meet an individual's health needs through communication and available resources to promote quality, cost-effective outcomes. Case managers are committed to enhancing the quality of care through the appropriate use of health care resources.
2007-2008 Mission Statement
As it has already been stated by the National Case Management Society of America the Atlanta Chapter's mission for 2007-2008 is "To positively impact and improve patient well being and healthcare outcomes". As a Chapter we will continually support our National Organization and our Local membership to achieve this mission. We will continue to promote increased membership and involvement of current members. We will promote the role and impact of Case Management within the Healthcare Profession and the Public Communities in which we serve. We will also continue to promote ongoing continuing education and professional development in regards to the role of case management.
