The Case Manager will be assigned to a dedicated customer team within the public sector and will be responsible for all aspects of care coordination, which include inpatient care advocacy, risk management and admissions counseling.  This rewarding role allows you to stay involved with patient care while exploring the business side of nursing.  Additionally, this position offers weekday hours with flexible schedules up to 7 p.m.* and major holidays off!

 

Primary responsibilities include:

  • Identifying members with unmet healthcare needs who are at risk for hospital admission/readmission
  • Making referrals to support treatment plans and improve self-management skills, as well as suggesting other interventions as appropriate
  • Following up with patients after discharge and helping to facilitate discharge plans to appropriate levels of care
  • Implementing risk management interventions
  • Assisting case managers with discharge planning
  • Completing risk assessments of members and offering appropriate community or outside resources to assist the members in obtaining care
  • Proactively working to reduce hospital admissions and readmissions

Minimum requirements include:

  • A current, unrestricted RN license in your state of residence
  • A clinical background that includes cardiac, diabetes, COPD, asthma or cancer

Those who also have one or more of the following will be preferred:

  • Experience in telephonic disease management and case management
  • Certification in case management
  • Certification in diabetes education 

*Some shifts require working until 7pm.
